Description

Even though Bacille Calmette-Guerin (BCG) is contraindicated in patients with HIV infection, it still may be given to small children in developing countries with a high rate of tuberculosis. A vaccinated child may develop an immune reconstitution inflammatory syndrome (IRIS) following the initiation of highly active antiretroviral therapy (HAART).

Criteria:

(1) HIV-infected child immunized with BCG

(2) asymptomatic for BCG disease prior to HAART therapy

(3) onset of BCG disease within 3 months of starting a HAART regimen

(4) immunological and/or viral evidence of immune reconstitution may or may not be present

BCG disease may be any of the types reported by Hesseling et al (see previous section):

(1) local disease

(2) regional disease

(3) distant disease

(4) disseminated disease
