Description

The larvae of Paragonimus may migrate into the extradural space of the spinal cord.


Migration may occur through perivascular or perineural tissues.

 

The larvae cause a granulomatous mass with hemorrhage.

 

Presentation:

(1) progressive weakness in the lower extremities

(2) paresthesias in the lower extremities

 

The mass can be seen onT2 weighted MRI, which shows intralesional septations.

 

Other clues:

(1) nodules in the lung

(2) eosinophilia in the peripheral blood

(3) positive serology for antibody to Paragonimus
