Description

von Gruenigen et al identified certain indicators that suggest that a woman with ovarian cancer may face a worsening prognosis. This can help to identify a patient who may benefit from palliative care. The authors are from University Hospitals Case Medical Center and Case Western Reserve University in Cleveland

Indicators of a declining course (downward trajectory):

(1) increasing hospitalizations for significant clinical events (SCE)

(1a) pleural effusion

(1b) malignant ascites

(1c) bowel obstruction

(2) short disease remissions (shorter time to disease recurrence)

(3) need for increased chemotherapy to control disease

A patient with a long initial remission tended to have a better prognosis.
