Description

Levine et al listed criteria for selecting an acutely ill patient for hospital-level care at home. A key element for success is proper patient selection. The authors are from Brigham and Women's Hospital, Harvard Medical School and Massachusetts General Hospital.


Patient selection:

(1) age >= 18 years

(2) no contraindications

(3) one or more indications (see below)

(4) operationally suitable

 

Conditions that may be suitable for hospital-level care at home:

(1) stable infection

(2) heart failure exacerbation

(3) COPD exacerbation

(4) asthma exacerbation

(5) chronic kidney disease requiring diuresis

(6) diabetes and its complications

(7) gout exacerbation

(8) hypertensive urgency

(9) previously diagnosed atrial fibrillation with rapid ventricular response

(10) stable venous thromboembolism or other condition requiring anticoagulation

(11) end-of-life desiring only medical management
