Jian et al listed indicators for reintubation of a patient following coronary artery bypass graft (CABG) surgery. The authors are from Shanghai First People's Hospital and Shanghai Jiaotong University.
Patient selection: extubation following CABG surgery
Indications for reintubation:
(1) severe dyspnea with respiratory rate > 35 breaths per minute
(2) increase in pulse and blood pressure compared to values recorded at the time of extubation
(3) PaO2 < 60 mm Hg (FIO2 not stated)
(4) PaCO2 > 50 mm Hg
(5) repeat arterial pH < 7.35 (with or without respiratoryacidosis)
(6) changes seen on chest X-ray (large area of atelectasis, large area of pneumonia)
where:
• Since pulmonary edema as a result of heart failure was an important reason for reintubation, evidence of increasing pulmonary edema could be added.
• A PaO2/FIO2 ratio might be a better metric.
