Description

Anseeuw et al listed indications for extracorporeal therapy for a patient with phenytoin poisoning. Extracorporeal therapy is only needed in severe cases because of the low incidence of irreversible tissue damage and mortality. The authors are members of the EXTRIP (Extracorporeal Treatments in Poisoning) workgroup.


Patient selection: phenytoin poisoning

 

Method of extracorporeal therapy:

(1) intermittent hemodialysis

(2) intermittent hemoperfusion

 

High protein binding limits the effectiveness of extracorporeal therapy.

 

Indications for extracorporeal therapy:

(1) prolonged coma

(2) prolonged, incapacitating ataxia

(3) any other sign of severe toxicity

 

Goal of therapy: clinical improvement
