Therapy with corticosteroids or other immunosuppressive agents should be considered in a patient with sarcoidosis when certain findings are present.
Indications for considering therapy in sarcoidosis:
(1) progressive pulmonary involvement with progressive respiratory symptoms, worsening pulmonary function, and increasing radiographic changes
(2) active cardiac involvement
(3) active central nervous system involvement
(4) posterior uveitis
(5) hypercalcemia and/or hypercalcuria
Therapy is not indicated just because the chest radiograph is abnormal.
