Discontinuation of opioid may be indicated in a chronic pain patient if certain conditions are present.
Indications for discontinuation of opioid therapy may include:
(1) diversion
(2) prescription forgery or fraud
(3) failure to adhere to the treatment plan
(4) lack of benefit
(5) need to convert a prescription for multiple opioids to a single agent (consolidation)
(6) serious adverse side effect(s)
(7) hospitalization
(8) excessive opioid dosing
