While quinine or its derivatives can be effective in controlling muscle spasms, they should only be used if the benefit outweighs the risk.
Indications for the use of quinine or a derivative to treat muscle cramps - all of the following:
(1) severe cramps that are disabling and difficult to control
(2) failure to relieve cramps by other approaches (exhausted alternatives)
(3) absence of contraindications to their use (allergy, past history of serious reaction to a quinine or a derivative in the past)
Requirements for use:
(1) The patient has been informed of the potential hazards and gives consent.
(2) The patient is carefully monitored for adverse effects.
(3) The drug is immediately discontinued if adverse effects are observed.
Serious adverse effects with quinine derivatives include:
(1) HUS-TTP
(2) DIC
(3) encephalopathy with coma
