Description

It is essential to perform a complete medical examination of a patient who presents with a drug overdose. Many significant findings can be missed if only a cursory exam is performed.


 

Sites that should be carefully examined:

(1) areas covered by hair

(2) areas covered by clothing

(3) axilla, groin and perineum

(4) vagina of a woman

(5) rectum

(6) mouth and oropharynx

 

Exposures that may be missed by an incomplete medical examination:

(1) fentanyl patches on perineum, in axilla or in mouth

(2) hidden injection drug sites

(3) intra-vaginal or intra-rectal drugs associated with body stuffing

 

Gastrointestinal exposure should be suspected in:

(1) small child

(2) body packer

(3) swallowing drugs when confronted by police

 

The complete exam may identify:

(1) unsuspected injuries

(2) findings suggestive of chronic use (such as dental changes seen with methamphetamines)
