The occurrence of fever in a patient with Brugada syndrome may impact the electrocardiogram.
In a patient with concealed Brugada syndrome fever may "unmask" a Brugada ECG pattern. Some patients with concealed Brugada syndrome may develop ventricular arrhythmias with severe or prolonged fever.
In a patient with overt Brugada syndrome fever may trigger ventricular arrhythmias (ventricular fibrillation, other) which can lead to sudden death.
Temperature sensitive mutations in SCN5A:
(1) Thr1620Met missense mutation: an elevated temperature causes the decay in the inward sodium current.
(2) F1344S
A mutation in other genes may be present.
Prompt treatment with an antipyretic can be lifesaving.
