Xanthogranulomatous pyelonephritis has a number of distinctive findings on imaging studies.
Classic radiology triad:
(1) unilateral large kidney
(2) renal pelvis stone(s)
(3) nonfunctioning or poorly functioning kidney
The kidney may show the "bear's paw sign" (large round "pad" with multiple "toes") on cross-sectional CT images. The "toes" are loculated cystic calyxes adjacent to the enlarged kidney ("pad").
