Opioids may be associated with a hypersensitivity reaction.
Types of reactions:
(1) histamine release from mast cells
(2) anaphylactoid reaction
(3) anaphylaxis
The reactions have been reported for: morphine, codeine, heroin, methadone, meperidine, fentanyl, remifentanil, buprenorphine, tramadol, dextromethorphan
Targets for the reaction
(1) opioid/opiate
(2) excipient
(3) contaminant (primarily in street drugs)
Clinical findings may include:
(1) flushing or generalized erythema
(2) urticaria
(3) bronchospasm
(4) initial tachycardia, followed by hypotension
