Description

Christenson et al reported the Hospital Observation Upon Reversal (HOUR) prediction rule for guiding management of a patient receiving naloxone for opioid reversal. The authors are from University of British Columbia in Vancouver.


Patient selection: emergency department patient requiring naloxone reversal

 

A patient with presumed opioid overdose reversed by naloxone can be safely discharged after 1 hour if all of the following are present:

(1) normal mobility

(2) normal oxygen saturation (> 95%)

(3) respiratory rate 11 to 19 breaths per minute

(4) temperature 35.1 to 37.4°C

(5) heart rate 51 to 99 beats per minute

(6) Glasgow Coma Scale 15

 

Performance:

• The sensitivity was 99% and specificity 40%.
