A hole on a walking surface that could cause fall injury requires a protective cover.
Features of a hole:
(1) gap or void in a walking surface (floor, roof, etc)
(2) minimum dimension >= 2 inches (5.1 cm)
Situations:
(1) construction site roadway or vehicular aisle
(2) hole on other construction walking surface

>= 2 times the maximum axle load of the largest vehicle that will cross the cover

>= 2 times the maximum weight likely to be imposed at any time (worker weight, equipment, materials, etc)
All covers should:
(1) be secured to prevent accidental displacement
(2) be color coded or labelled ("hole", "cover") to warn of the hazard
Exception: hole in a street or road outside of the construction site with cast iron manhole cover or street grate.
