Description

Hypothyroid myopathy may have a number of clinical presentations, one of which is Hoffmann’s syndrome.


 

Clinical features of Hoffman’s syndrome:

(1) hypothyroidism

(2) muscular pseudohypertrophy

(3) progressive proximal muscle weakness

(4) slow movements

(5) painful muscle spasms and cramps

(6) stiff muscles with delayed muscle relaxation which did not improve on repetition

(7) reduced deep tendon reflexes

(8) clinical improvement with thyroxine replacement

 

Laboratory findings:

(1) elevated muscles enzymes (CK, etc), often marked

(2) low serum concentration of thyroxine

 

Electromyography:

(1) repetitive positive waves
