The relative risk of a person with human immunodeficiency virus (HIV) infection transmitting the virus to an HIV-negative partner can be estimated based on the nature of sexual practice.
Parameters:
(1) sexual practice
(2) use of condom protection
Sexual Act |
Relative Risk |
insertive fellatio |
1 |
receptive fellatio |
2 |
insertive vaginal sex |
10 |
receptive vaginal sex |
20 |
insertive anal sex |
13 |
receptive anal sex |
100 |
The failure to use a condom increases the relative risk by 20 (relative risk with condom use is 1).
where:
• The risk estimates for vaginal and anal sex are from the European Study Group. This involved heterosexual couples in stable relationships. Homosexual and unstable couples might have higher rates.
• The presence of concurrent factors (other sexually transmitted disease, ulcerations, other immunosuppression) could increase the risk.
• A small breach in a latex condom might alter the risk estimate.
According to the CDC method of estimation:
cumulative risk for each sexual act =
= (relative risk for sexual act) * (relative risk for condom use)
Purpose: To estimate the relative risk of an HIV-positive person transmitting the virus to an HIV-negative partner during a sex act.
