Grading of urothelial carcinomas can be done based on histologic and cytologic features.

Histology

Score

well-differentiated papillary carcinoma, tumor cell layers >= 7

0

well to moderately differentiated papillary carcinoma, tumor cell layers > 7

1

papillary carcinomas with > 20 tumor cell layers, or solid carcinoma

2

poorly differentiated urothelial solid carcinomas with or without squamous or small cell components

3

Cytology

Score

slight nuclear atypia without nucleoli

0

moderate nuclear atypia with nucleoli of small to moderate size, mainly in the central position

1

moderate to marked atypia with mononuclear giant cells. Multiple predominantly eccentrically located nucleoli
