Description

Sarcomatoid carcinoma of the lung refers to a group of poorly differentiated lung tumors with a spectrum of histologic features.


 

In general a sarcomatoid carcinoma consists of:

(1) a non-small cell lung carcinoma (NSCLC) component, often poorly differentiated

(2) sarcoma (rhabdo, osteo, chondro) or sarcomatoid (spindle cells, anaplastic giant cell) component

 

Subtypes:

(1) pleomorphic carcinoma

(2) spindle cell carcinoma

(3) giant cell carcinoma

(4) carcinosarcoma

(5) pulmonary blastoma

 

Pleomorphic carcinoma:

(1) >= 10% malignant spindle cells and/or anaplastic giant cells

(2) remainder (may be 0%) consists of a NSCLC (squamous, adeno, large cell, mixed)

 

Spindle cell carcinoma:

(1) carcinoma with all malignant spindle cells

 

Giant cell carcinoma:

(1) carcinoma with all anaplastic giant cells

 

Carcinosarcoma:

(1) component of NSCLC (squamous, adeno, large cell, mixed)

(2) sarcoma component (rhabdo, osteo, chondro, other)

 

Pulmonary blastoma - a biphasic tumor:

(1) primitive adenocarcinoma

(2) primitive mesenchymal stroma

(3) variable sarcoma component (rhabdo, osteo, chondro, other)
