Gastric intestinal metaplasia may be classified based on its histologic features.
Parameters:
(1) crypt architecture
(2) cell types
(3) mucins
Crypt Architecture
Cell Types
Mucins
Type
organized with straight crypts
round and more uniform goblet cells; columnar cells and Paneth cells present
goblet cells contain sialomucins; columnar cells contain no mucin
I (complete)
architectural distortion
variable goblet cells; reduced columnar or Paneth cells
goblet cells may contain sialo or sulfomucin; columnar cells may contain sialomucin
II (incomplete)
architectural distortion with branched glands at base
variable goblet cells; absent Paneth cells
goblet cells may contain sialo or sulfomucins ;columnar cells predominantly show sulfomucins;
III (incomplete)
The risk of cancer increases with increasing type (Type III > Type II > Type I).
