Description

Riley et al evaluated colon biopsies from patients with ulcerative colitis based on the histologic features. They correlated this with the level of disease activity. The authors are from the University of Manchester, Hope Hospital and Bury General Hospital in Manchester, England.

Parameters:

(1) acute inflammatory cell infiltrate in the lamina propria (LP)

(2) chronic inflammatory cell infiltrate in the lamina propria

(3) crypt abscesses

(4) mucin depletion

(5) breaches (lack of integrity) of the surface epithelium

(6) irregularities in crypt architecture

Findings for Each Histologic Feature

Points

total score =

= SUM(points assigned to all 6 parameters)

Interpretation:

• minimum score: 0

• maximum score: 18

• The higher the score the more active the ulcerative colitis.

Relapse rate was associated with:

(1) acute inflammatory cell infiltrate

(2) crypt abscesses

(3) mucin depletion

(4) breaches of the surface epithelium
