Description

Some acinic cell carcinomas of the parotid gland are aggressive tumors with a poor prognosis while others are indolent with a relatively good prognosis. Gomez et al identified histologic features associated with a poor outcome. The authors are from Memorial Sloan-Kettering Cancer Center in New York City.


 

Patient selection: acinic cell carcinoma of the parotid gland

 

Criteria for a high grade acinic cell carcinoma - one or more of the following:

(1) high mitotic activity (> 2 mitoses per 10 hpf)

(2) tumor necrosis

(3) pleomorphic nuclei

 

Criteria for an aggressive acinic cell carcinoma - one or more of the following:

(1) extracapsular extension

(2) positive surgical margins

(3) one or more high grade criteria (see above)

 

The presence of aggressive features was associated with a worse disease-free survival and overall survival.

 

Additional factors associated with a worse overall survival:

(1) atypical mitoses

(2) diameter > 4 cm
