Description

The Hepatic Artery Resistance Index (HARI) can be predictive of outcome in children with biliary atresia. As hepatic fibrosis progresses there is increased intrahepatic vascular resistance.


 

Patient selection: child with biliary atresia

 

Method: Doppler ultrasonography with monitoring of the hepatic artery at the porta hepatis

 

Measurements:

(1) peak systolic blood velocity in cm/s

(2) end-diastolic blood velocity in cm/s

 

HARI =

= ((peak systolic velocity) - (end-diastolic velocity)) / (peak systolic velocity)

 

With abnormal hepatic artery flow, there is reversal of flow during diastole, resulting in negative end-diastolic velocities. Subtraction of a negative score becomes additive.

 

Interpretation:

• A HARI < 1.0 was associated with a better prognosis than a HARI >= 1.0
