Description

Rotem et al listed several hematologic biomarkers that may help to identify a patient with an infection following cesarean section. The authors are from Hebrew University and Ben-Gurion University in Israel.

Patient selection: status post cesarean section

Parameters during the first 24 hours after surgery:

(1) white blood cell count

(2) absolute neutrophil count

(3) absolute neutrophil count

(4) NLR (neutrophil to lymphocyte ratio)

(5) PLR (platelet to lymphocyte ratio

Parameter

Infection

No Infection

WBC count

15.08 +/- 6.16 (x10^9)

13.27 +/- 4.23 (x10^9)

absolute neutrophil

12.64 +/- 5.56 (x10^9)

10.79 +/- 3.90 (x10^9))

absolute lymphocyte

1.38 +/- 0.59 (x10^9)

1.59 +/- 0.57 (x10^9)

NLR

11.14 +/- 9.60

7.76 +/- 4.32

PLR

176.52 +/- 106.34

149.90 +/- 69.85

All of the biomarkers show overlap in their ranges, limiting the discriminatory ability. An elevated value may be associated with infection, but a lower value may not rule it out.

The area under the ROC curve is 0.67.

One approach to use the information is to create cut-offs, above which the patient should be screened for infection. This would be one of several screens looking for infection.
