The Hannover classification for cardiac allograft rejection can be used to describe non-acute histologic features in an endomyocardial biopsy from a patient with a cardiac transplant. The classification is a modification of the Billingham scheme. The authors are from Hannover Medical School in Lower Saxony.

Features of chronic rejection:
(1) interstitial and partly endocardial aggressive fibrosis
(2) persisting variable vasculopathy
Features of slight vasculopathy:
(1) endothelial swelling with degenerative changes
(2) slight perivascular mantle infiltrates
Features of severe vasculopathy:
(1) vascular necrosis
(2) massive intramural infiltrates
(3) endovasculopathy or thrombotic vasculopathy
