Description

The Hannover classification for cardiac allograft rejection can be used to describe non-acute histologic features in an endomyocardial biopsy from a patient with a cardiac transplant. The classification is a modification of the Billingham scheme. The authors are from Hannover Medical School in Lower Saxony.


 

Non-acute categories of change:

(1) chronic rejection (C)

(2) vasculopathy (B)

(3) fibrosis (F)

(4) non-rejection changes

Features of chronic rejection:

(1) interstitial and partly endocardial aggressive fibrosis

(2) persisting variable vasculopathy

 

Features of slight vasculopathy:

(1) endothelial swelling with degenerative changes

(2) slight perivascular mantle infiltrates

 

Features of severe vasculopathy:

(1) vascular necrosis

(2) massive intramural infiltrates

(3) endovasculopathy or thrombotic vasculopathy

 

Description of fibrosis may be by:

(1) localization

(1a) endocardial (e)

(1b) aggressive endocardial (ea)

(1c) interstitial (i)

(1d) aggressive interstitial (ia)

(2) percent distribution
