Keep and Manford determined the appropriate size of an endotracheal tube to use in a child based on age, weight and height. The authors felt that selection based on height was the most accurate of the three. The authors are from St. George's Hospital in London and Queen Mary's Hospital for Children in Surrey.
Parameters:
(1) age
(2) weight in kilograms
(3) height in centimeters

In Table 1 (page 182) the mean age, weight and height are listed for each endotracheal tube diameter. Equations from Minitab that approximates this data are:
endotracheal tube diameter in mm =
= (-0.006545 * ((age in years)^2)) + (0.3913 * (age in years)) + 3.736
endotracheal tube diameter in mm =
= (-0.001821 * ((weight in kg)^2)) + (0.1993 * (weight in kg)) + 2.521
endotracheal tube diameter in mm =
= (0.000067 * ((height in cm)^2)) + (0.02882 * (height in cm)) + 1.710
