Description

Litovitz et al reported guidelines for managing a pediatric patient who has swallowed a disc battery. One group of patients can be managed conservatively. The authors are from Georgetown University, George Washington University, and University of Virginia.


 

Patient selection: disc battery ingestion known or suspected

 

Patient who can be managed by carefully monitoring – all of the following:

(1) The patient is > 12 years old.

(2) The patient swallowed only 1 battery.

(3) The battery size is definitely <=12 mm.

(4) No magnet was swallowed

(5) The patient does not have any pre-existing esophageal disease.

(6) The patient is currently asymptomatic.

(7) The patient has had no symptoms since the ingestion.

(8) The patient and caregiver are reliable, mentally competent and able to promptly return.

 

A patient who fail one or more of these items must be managed more aggressively.

 

Home management:

(1) Eat a regular diet.

(2) Encourage activity.

(3) Confirm battery passage in stool.

 

Return for re-evaluation:

(1) If any symptoms develop.

(2) If the battery has not been passed by 10-14 days.
