Description

Omalu et al reported a contusion index to describe cerebral contusions at autopsy. This requires grading of contusion injuries in various lobes and regions of the brain. The authors are from the Allegheny County Coroner's Office and Abakaliki Teaching Hospital (Nigeria).

This is a modification of the grading schema of Adams et al.

Parameters:

(1) surface contusions

(2) maximum parenchyma depth of contusions

Parameter

Finding

Points

surface contusion

no contusion

0

1 gyri or 2 adjacent gyri on one surface of the lobe

1

much of 1 surface of the lobe

2

involve more than 1 surface of the lobe (extensive or focal)

3

parenchymal depth

no contusion

0

leptomeninges with subarachnoid hemorrhage

1

partial or full cortical thickness

2

involves interdigitating or immediate subcortical white matter

3

extends to deep or periventricular white matter

4

score for a lobe or region =

= (grade for surface) * (grade for parenchymal depth)

Interpretation:

• minimum score: 0

• maximum score: 12
