Description

Ross et al modified the Geriatric Trauma Outcome Score (GTOS) with development of the GTOS III score. This can help to identify a patient who may require more aggressive management. The authors are from Carolinas Medical Center in Charlotte and University of Texas Southwestern Medical Center in Dallas.

Patient selection: age >= 65 years of age

Outcome: 1-year mortality

Parameter:

(1) age in years

(2) ISS

(3) transfusion of a blood product within the first 24 hours

(4) Glasgow coma scale (GCS)

(5) discharge (adverse if to skilled nursing facility, long-term acute care hospital or inpatient hospice)

GTOSIII =

= SUM(points for all 5 parameters)

Interpretation:

minimum score: 65

maximum score: depends on the ISS and age

A score above 184 is associated with a very high mortality rate.

X =

= (0.06147 * (score)) - 8.5552

probability of 1-year mortality =

= 1 / (1+EXP((-1) * X))

Performance:

The area under the ROC curve is 0.88.
