Description

Blaser et al reported the Gastrointestinal Dysfunction Score (GIDS) for evaluating gastrointestinal function in a critically-ill patient. The authors are from multiple institutions in Europe and Argentina.


Patient selection: critically-ill adult

 

Parameters:

(1) bowel sounds

(2) vomiting

(3) gastric residual volume (GRV) in mL

(4) GI paralysis or dynamic ileus

(5) abdominal distension

(6) diarrhea

(7) gastrointestinal bleeding

(8) intra-abdominal pressure in mm Hg

(9) oral intake

(10) mesenteric ischemia

 

GI Dysfunction

Grade

no risk of GI dysfunction

0

increased risk for GI dysfunction

1

GI dysfunction present

2

GI failure

3

life-threatening GI dysfunction

4

 

Grade 4: one or more of the following:

(7) GI bleeding with hemorrhagic shock

(10) mesenteric ischemia

(8) abdominal compartment syndrome

 

Grade 3: 3 or more of the following:

(4) prokinetic use

(4) GI paralysis or dynamic ileus

(5) abdominal distension

(6) severe diarrhea

(7) GI bleeding requiring transfusion

(8) intra-abdominal pressure > 20 mm Hg

 

Grade 2: 1 or 2 of the following

(6) severe diarrhea

(7) GI bleeding requiring transfusion

(8) intra-abdominal pressure > 20 mm Hg

 

Grade 2: 3 or more of the following:

(9) no oral intake

(1) absent bowel sounds

(2) vomiting

(3) gastric residual volume > 200 mL

(4) GI paralysis or dynamic ileus

(5) abdominal distension

(6) diarrhea, not severe

(7) GI bleeding not requiring transfusion

(8) intra-abdominal pressure 12 to 20 mm Hg

 

Grade 1: 2 of the following:

(9) no oral intake

(1) absent bowel sounds

(2) vomiting

(3) gastric residual volume > 200 mL

(4) GI paralysis or dynamic ileus

(5) abdominal distension

(6) diarrhea, not severe

(7) GI bleeding not requiring transfusion

(8) intra-abdominal pressure 12 to 20 mm Hg

 

Grade 0: 1 of the following with oral intake

(1) absent bowel sounds

(2) vomiting

(3) gastric residual volume > 200 mL

(4) GI paralysis or dynamic ileus

(5) abdominal distension

(6) diarrhea, not severe

(7) GI bleeding not requiring transfusion

(8) intra-abdominal pressure 12 to 20 mm Hg

 

Grade 0: none of the above and no symptoms
