Chen et al reported the FRAIL-J score for screening elderly Japanese adults for frailty. The authors are from Kyushu University and Shimane University in Japan.
Patient selection: >= 65 years of age in Japan
Parameters:
(1) fatigue (F): feel that everything done is an effort without a reason in the past 2 weeks
(2) resistance (R): climbing stairs without using the handrail or wall for support
(3) ambulation (A): difficulty walking 1 km without resting and without walking aids by self
(4) illness (I): hypertension, diabetes, cancer (excluding minor skin cancer), chronic lung disease, heart attack, congestive heart failure, angina, asthma, arthritis, stroke, kidney disease
(5) loss of weight (L): unintentional weight loss > 2-3 kg in the past 6 months

• A cutoff of >= 2 can be used to identify a patient who may be frail.
• An additional evaluation is needed to make the diagnosis since the PPV is low (13%).
Performance:
• The area under the ROC curve is 0.86.
• The negative predictive value is 99.7% so is good for exclusion.
