Gathman et al described the occurrence of otorrhea in infants and small children with cleft palate related to reflux of formula into the middle ear. The authors are from the University of Minnesota and Children's Minnesota Hospital.
Patient selection: cleft lip and palate
Mechanism: reflux of formula into the middle ear
Clinical features:
(1) history of chronic otitis media
(2) status post placement of pressure equalizing (PE) tubes
(3) onset of otorrhea after feeding on breast milk or formula
(4) resolution after repair of cleft palate
The volume of fluid is low, but analysis should show shared features with the formula.
