Description

Li et al reported a measure of infarct size in a patient with acute pontine infarction. The extent of pontine infarction along the conduction tract may contribute to clinical deterioration. The authors are from the Third Affiliated Hospital of Sun Yat-Sen University in Guangzhou and the First Affiliated Hospital of Clinical Medicine of Guangdong Pharmaceutical University.


Patient selection: acute pontine infarct

 

Early neurological deterioration (END) - at least one of the following:

(1) incremental increase in NIHSS by >= 1 point in motor power within the first week after admission

(2) incremental increase in NIHSS by >= 2 point in total score within the first week after admission

 

MRI: axial diffusion-weighted imaging on sagittal T2

 

Parameters on brain MRI (see Figure):

(1) ventrodorsal length in cm

(2) thickness in cm

 

measure of infarct size = MLMT =

= (ventrodorsal length) * (thickness)

 

Interpretation:

The MLMT ranged from 0.084 to 8.321 in the study population.

The higher the MLMT the greater the risk of END (odds ratio 4.6).
