Description

Lee et al developed a simple flow diagram to decide which method to use when performing a wedge resection of a gastric submucosal tumor. The authors are from Korea University in Seoul.

Patient selection: submucosal tumor of stomach (majority GI stromal tumors or GIST)

Parameters:

(1) size in cm

(2) circular location in the stomach

(3) growth patttern

A transgastric wedge resection was performed if all of the following were present:

(1) diameter <= 2.5 cm (1 inch)

(2) circular location low or high in hidden area

(3) endophytic growth

where:

• The hidden area includes the posterior wall of the stomach or lesser curvature.

If the patient did not meet criteria for the transgastric approach then an exogastric wedge resection was performed.
