Description

Many cases of pediatric intussusception can be reduced by nonsurgical means. The presence of certain findings on abdominal ultrasound may identify a patient who requires surgical reduction.


Patient selection: pediatric patient with intussusception

 

Evaluation: abdominal ultrasound

 

Findings associated with failed nonsurgical reduction of the intussusception:

(1) thick peripheral hypoechoic rim

(2) free intraperitoneal fluid

(3) fluid trapped within the intussusception

(4) enlarged lymph node

(5) pathologic leading point

(6) absence of blood flow in the intussusception

(7) bowel wall thickening

= SUM(findings present)

 

Interpretation:

• minimum score: 0

• maximum score: 7

• The presence of one or more findings is associated with failure of nonsurgical interventions and with the need for surgery.
