Description

A number of clinical and laboratory findings may be associated with a poor prognosis for a patient with chronic lymphocytic leukemia (CLL).


 

Patient selection: CLL

 

Clinical findings associated with a poor prognosis:

(1) advanced age

(2) male gender

(3) advanced stage at diagnosis

(4) diffuse pattern of bone marrow infiltration

(5) poor response to therapy OR short duration of response to therapy

 

Laboratory findings associated with a poor prognosis:

(1) high serum beta-2 microglobulin concentration

(2) high level (>= 30% of CLL cells) expression of surface CD38

(3) high level (>= 30% of CLL cells) expression of cytoplasmic ZAP-70

(4) loss or mutation of TP53

(5) unmutated IGHV gene

(6) lymphocyte doubling time < 12 months

(7) 17p13 deletion

(8) 11q23 deletion

(9) complex cytogenetic abnormalities

(10) increased interleukin 8 or TNF-alpha (cytokines)

(11) high Ki-67 index

(12) high level of expression of lipoprotein lipase

(13) decreased micro-RNA expression

(14) elevated serum thymidine kinase
