Tavaglione et al reported a the Fibrotic NASH Index (FNI) to screen for fibrotic nonalcoholic steatohepatitis (NASH). These can help to identify a patient who should be a more extensive workup. The authors are from multiple institutions in Italy, Sweden, Finland, and the United States.
Patient selection: high risk for NASH (obese, type 2 diabetes and metabolic syndrome)
Parameters:
(1) serum AST in U/L
(2) hemoglobin A1c in percent
(3) serum HDL cholesterol in mg/dL
Parameter
Points
serum AST
2.54 * LN(AST)
hemoglobin A1c
3.86 * LN(HgbA1c)
serum HDL cholesterol
-1.66 * LN(HDL)
value of X =
= SUM(points for all of the parameters) - 10.33
probability of fibrotic HASH =
= 1 / (1 + EXP((-1) * X))
Performance:
• The area under the ROC curve was 0.78.
