Description

Oh et al evaluated thermally-burned patients for inhalation injury. The authors are from Fort Sam Houston in Texas.


Patient selection: thermal burn

 

Outcomes: death, acute lung injury (ARDS) or pneumonia

 

Parameters:

(1) findings seen on fiberoptic bronchoscopy

(2) total RADS score from the chest CT

(3) number of slices in the chest CT scan

 

The total RADS score is the total score of all the subscores for each slice of the CT scan.

 

RADS score per slice =

= (total RADS score) / (number of slices)

 

Findings on bronchoscopy associated with inhalation injury:

(1) carbonaceous deposits

(2) mucosal erythema

(3) mucosal sloughing

(4) mucosal ulcerations

 

The risk of a poor outcome was increased when both of the following are present:

(1) inhalation injury on bronchoscopy

(2) RADS score per slice > 8

 

The presence of both findings had an odds ratio of 12.7.
