Description

Low et al reported the FAM-FACE-SG score for evaluating patients who have frequent hospital admissions. This can help to identify high risk patients. The authors are from Singapore General Hospital and Singapore Health Services.

FAM-FACE-SG is based on the first letter of the 9 parameters.

Parameters:

(1) furosemide (F)IV therapy during index admission

(2) admissions in past year (A)

(3) Medifundassistance needed (M, a fund in Singapore to help pay medical bills)

(4) frequent ED use (F) in 6 months before index admission

(5) anti-depressant therapy in past year (A)

(6) Charlson Comorbidity Index (C)

(7) end-stage renal disease on dialysis (E)
