Description

Hagino et al reported a Fall Risk Assessment Score for identifying a hospitalized patient who is at increased risk for fall in the hospital. The authors are from Kofu National Hospital and University of Yamanashi in Japan.


Patient selection: hospitalized patient

 

Parameters:

(3) period of rapid change (recovery or deterioration) in diagnosis or disability

(4) number of days since admission or bed change

(5) starting rehabilitation or training

(6) tendency to act without pressing the nurse call button

(7) strength of leg

(8) mobility

(10) bedridden status

(11) impaired memory or judgment

(12) use of wheelchair, walker or handrail (mobility assistance)

(13) need for assistance with mobility and excretion (personal ADL)

(14) uses sleep medication

(15) uses anti-Parkinsonian drug or muscle relaxant

(16) uses a narcotic
