Description

Spencer et al reported factors associated with outcomes in pediatric patients with early-onset short bowel syndrome (SBS). These can help to identify a patient who may require more aggressive management including intestinal transplantation. The authors are from the University of Michigan and Mercy Children's Hospital in Toledo.

Patient selection: infant or child with short bowel syndrome with onset at <= 6 weeks of age

Risk factors for mortality:

(1) small bowel length as percent of that predicted for age < 10% (RR 5.7)

(2) cholestasis, with conjugated serum bilirubin >= 2.5 mg/dL (RR 22.7)

Predictors of weaning from parenteral nutrition:

(1) small bowel length as percent of that expected >= 10% (RR 11.8)

(2) presence of ileocecal valve (RR 3.9)
