Description

Roelfsema et al evaluated a variety of factors affecting the patient following surgery on a pituitary adenoma. The authors are from Leiden University Medical Center in The Netherlands.


 

Patient selection: pituitary adenoma following surgical remission

 

Types of pituitary adenomas:

(1) acromegaly (growth hormone)

(2) Cushing's disease (ACTH)

(3) prolactinoma

(4) non-functioning

 

Factors impacting recurrence:

(1) The recurrence risk was highest with prolactinoma and lowest with nonfunctioning adenomas.

(2) The recurrence risk peaks at 1 to 5 years after surgery.

(3) For functioning adenomas the risk of recurrence was low if the post-operative serum hormone level was low.

 

The risk of impaired pituitary hypofunction is:

(1) highest with Cushing's disease.

(2) associated with more extensive surgery.
