Description

Puvanesarajah et al evaluated factors affecting perioperative blood transfusion in adults undergoing surgery to correct a spinal deformity. These can help to identify a patient who may benefit from an intervention to reduce the need for surgery. The authors are from the Johns Hopkins University and University of Virginia.


Patient selection: adult undergoing surgery to correct spinal deformity (adult spinal deformity, ASD) that involved 4 or more levels

 

Risk factors for intraoperative blood transfusion volume:

(1) 3 column (anterior, middle, posterior) osteotomy (complex surgery)

(2) preoperative hemoglobin < 11.5 g/dL

(3) prolonged operative times

 

Risk factors for postoperative blood transfusion (up to 10 days after surgery):

(1) history of smoking

(2) failure to use intraoperative antifibrinolytic drugs

(3) greater volume of intraoperative transfusion using blood salvage (0.39 unit increase per intraoperative unit)

(4) lower volume of allogeneic blood transfused (0.23 unit decrease per allogeneic unit)
