Description

Panaccione et al reported factors associated with high-risk Crohn's disease. These can help to identify a patient who requires more aggressive management. The authors are from the Canadian Association of Gastroenterology and are from multiple institutions in Canada.


Patient selection: Crohn's disease

 

Outcome: relapse, surgery or complicated course

 

Factors associated with high risk:

(1) younger age at onset (age < 40 years used elsewhere)

(2) smoking

(3) longer duration of the disease

(4) early therapy with corticosteroids

(5) presence of fistulizing perianal Crohn's disease

(6) history of previous intestinal resection (? stricturing disease)

(7) disease located in rectum, jejunum or upper GI tract

(8) extensive disease

(9) anemia (low hemoglobin)

(10) low serum albumin

(11) high serum C-reactive protein

(12) high fecal calprotectin concentration

(13) deep ulcerations seen on endoscopy

 

If none of these factors are present, then the patient is low risk.

 

If multiple factors are present, then the patient is high risk.
