Description

Ligation of the dorsal penile vein may be effective therapy in carefully selected patients with erectile dysfunction (ED). Cakan et al listed selection criteria for the procedure. The authors are from the SSK Diskapi Training Hospital in Turkey.

Surgical procedure: ligation of the dorsal vein of the penis

Surgery was performed on patients with evidence of veno-occlusive dysfunction, with venous incompetence allowing leakage of blood into the intermediate and deep venous systems.

Factors associated with a better outcome:

(1) age of the patient < 40 years

(2) secondary erectile dysfunction

(3) duration of ED < 2 years

(4) non-smoker

(5) nondiabetic

(6) site of leakage distal

The more factors present the better the outcome. A patient with no or few factors is likely to have a disappointing outcome.

Complications:

(1) surgical site infection

(2) penile edema, which may be transient or prolonged

(3) painful erection

(4) penile curvature
