Extracorporeal membrane oxygenation (ECMO) therapy can be used to manage a neonate with the meconium aspiration syndrome (MAS). The respiratory problems (including pulmonary hypertension) of MAS are potentially reversible.
Patient selection: meconium aspiration syndrome
ECMO: venovenous (VV)
Indications for ECMO - all of the following:
(1) severe meconium aspiration syndrome
(2) oxygenation index > 40
(3) mean airway pressure (Paw) >= 20 cm H2O
(4) weight > 2 kilograms
A neonate with a Paw <= 15 cm H2O can be managed with conventional therapy
