Description

Pulmonary hemorrhage may rarely occur after exercise.


Patient selection: recent exercise

 

Risk factors:

(1) status post an episode of maximal exertion (marathon, handball, etc)

(2) underlying lung disease (amyloidosis, cystic disease, etc)

(3) acute pulmonary congestion

(4) coagulation disorder

 

Clinical findings:

(1) shortness of breath

(2) dry cough

(3) hypoxemia

(4) hemoptysis

 

Examination of sputum may show hemosiderin-laden macrophages.

 

The diagnosis requires exclusion of other causes including Goodpasture's disease, Wegener's granulomatosis, pulmonary embolism and acute heart failure.
