Description

A patient with peripheral arterial disease (PAD) may develop an inflammatory response in association with exercise. Often continuation of an exercise program will improve perfusion and reduce this response.


 

Patient selection: patient with severe peripheral arterial disease (PAD)

 

Mechanism: ischemia with or without reperfusion injury

 

Clinical and laboratory features:

(1) There is an increase in the white blood cell count temporally related to exercise.

(2) It tends to occur when the exercise results in claudication.

(3) It may be accompanied by exercise-induced pain.

(4) It may be associated with microalbuminuria.

(5) C-reactive protein (CRP) may be elevated.

 

Diagnosis requires exclusion of other explanations for the leukocytosis or inflammatory response (infection, drugs, etc)..
