Exercise-associated postural hypotension (EAPH) can occur in some athletes following vigorous exercise.
Features of exercise-associated postural hypotension:
(1) drop in systolic blood pressure in upright position relative to supine
(1a) > 30 mm Hg
(1b) >= 20 mm Hg AND symptoms (including syncope)
(2) associated with exercise
If the patient is symptomatic then symptoms are relieved if the patient becomes supine.
Risk factors:
(1) autonomic dysfunction
(2) prolonged exertion
(3) drugs
(4) dehydration
Differential diagnosis:
(1) heat injury
(2) post-exercise hypotension
