Description

Reconstituting and administering an injectable drug may be associated with a number of errors.


 

Errors in reconstituting an injected medication:

(1) wrong diluent

(2) wrong amount of fluid added (too much or too little)

(3) failure to mix adequately

(4) failure to handle particulate material properly

(5) failure to handle foaming properly

(6) failure to handle color properly

(7) reconstitution of an outdated drug

(8) bacterial or fungal contamination

(9) leakage

(10) improper storage

 

Errors in administering a reconstituted medication:

(1) injection of contaminated solution

(2) delivery by wrong route

(3) failure to use an appropriate filter

(4) mixing with an incompatible solution

(5) administration of a reconstituted drug that is outdated

(6) incorrect volume

 

where:

• A medication that has been reconstituted needs to be administered within a set period of time defined by the manufacturer or determined by storage conditions.

• Foam, color and/or particular material may or may not be significant. An error can occur when there is a failure to respond to a change or when there is an overreaction.
